How to check a Fort Worth-area facility's record yourself

Texas HHSC's Long Term Care Regulatory division maintains license status and survey/enforcement history for every licensed assisted living facility in the state. Call HHSC's Complaint and Incident Intake line at 1-800-458-9858, available 24 hours a day, to ask about a specific facility's complaint or inspection history, or to file a new complaint.

Assisted living facilities are not rated by Medicare's Care Compare star system -- that tool covers nursing homes only. A facility's HHSC survey record, not its online reviews, is the closest thing Texas has to an official quality record for assisted living.

When you do get a facility's record, read the deficiency narratives rather than counting citations. A missing signature and an injury to a resident can appear in the same document, formatted almost identically. Look for the same finding repeating across consecutive surveys -- that is a pattern, not an isolated incident.

What to actually look at once you are in there

Pulling the record is easy. Reading it usefully is the part nobody explains.

  • Match the license type to your parent's needs, not just to today. Ask directly about admission and retention criteria.
  • Check the licensee name against the operator. Ownership turns over regularly, and buildings are often marketed under a brand that differs from the license holder.
  • Read deficiency narratives, do not count citations. A missing signature and an injured resident can appear in the same document, formatted identically.
  • Look for repetition across surveys. One citation is an incident. The same finding across consecutive surveys is a pattern.

What happens after I report suspected abuse to APS?

An APS caseworker reviews the report and, for situations that meet the threshold for investigation, typically responds within a set timeframe based on the severity described, ranging from immediate response for emergencies to several days for lower-risk reports.

My parent is being discharged from a Fort Worth hospital in 72 hours. What do I do first?

Get the discharge plan in writing from the hospital's case manager or social worker, confirm whether your parent was formally admitted as an inpatient or held under observation status, and ask specifically which senior care facilities have already said yes to the referral -- not just which ones the hospital contacted.

What's the difference between inpatient admission and observation status for Medicare purposes?

Medicare's skilled nursing facility benefit requires a qualifying inpatient hospital admission. Time spent under 'observation status' does not count toward that requirement, even if your parent physically stayed overnight in a hospital bed for multiple nights.
