Dad is being discharged in 72 hours

The fastest safe path when a hospital case manager gives you days, not weeks.

Quick answer

The fastest safe path when a hospital case manager gives you days, not weeks.

HomeBy SituationDad is being discharged in 72 hours

A case manager gave you a date. Everything below assumes you have days, not months, and that nobody has explained what your options actually are.

Short answer

The fastest safe path when a hospital case manager gives you days, not weeks.

What to do, in order

  1. Get the discharge plan in writing. Ask the case manager or social worker for it on paper.
  2. Confirm inpatient versus observation status. Ask directly, and ask in writing. Medicare's skilled nursing benefit requires a qualifying inpatient admission, and observation nights do not count no matter how many were spent in a hospital bed.
  3. Ask which facilities actually accepted the referral. Case managers send inquiries to many communities and hear back from few. The list of places that have said yes is the only list that matters today.
  4. Push on the discharge date. It is usually less fixed than it sounds, particularly when no safe plan exists yet.
  5. Verify the license before you sign. A few minutes on your state's licensed-facility search tool, even inside a 72-hour window.

Questions families ask

What's the difference between Texas's Medicaid expansion and Medicaid for seniors?

Texas has not expanded Medicaid under the Affordable Care Act, but that decision has no bearing on long-term-care Medicaid for seniors, which is a completely separate, decades-old eligibility pathway for people who are 65 or older, or blind or disabled.

How does a Fort Worth family apply for the STAR+PLUS HCBS waiver?

Apply through Your Texas Benefits online, or call Texas 2-1-1 and ask for the aging and disability services option. A Texas HHSC caseworker will complete a nursing-facility level-of-care assessment as part of determining eligibility.

What are Texas's nursing facility Medicaid income and asset limits in 2026?

For 2026, the special income limit for Texas nursing facility Medicaid is $2,901/month, with a $2,000 asset limit for a single applicant. Income above the limit generally requires a Miller Trust (Qualified Income Trust) to still qualify.
